This questionnaire will help you review your personal impact or for a colleague to review his or her personal impact.
Your answers will be treated in strictest confidence. No individually identifiable data will be seen by the person concerned, only aggregate scores appear in the final results. Any written comments you make will appear without any name or identification attached.

Because this is a demonstration version, we use dummy data for your five colleagues, who would normally complete the profile questionnaire too. Also, you also normally only complete the questionnaire when at least three of your nominated colleagues have completed their input.
